Name Changes

Generally, you may change your name if you have never been arrested or adjudicated guilty of a crime, have never been adjudicated bankrupt, do not have any money judgments against you and are not seeking to change your name for any ulterior or illegal purpose. At Sandy T. Fox, P.A., your petition for name change will be filed in the county where you reside.

A person who wishes to change their name must file a petition to do so in the county which he or she resides. The petition must include the following information: petitioner’s present name and the name to be changed to; his or her current residence, as well as all residences since birth; his or her birth information and biological parent’s names; names of all family members; current occupation, including the address, as well as all positions held over the past five years; education; criminal history; judgments against him or her if applicable; and finally the purpose for the name change. You must also obtain and file with the court a state and federal background check along with fingerprints from a licensed provider from the Florida Department of Law Enforcement.

Once your petition for name change has been filed, your case can be set for a final hearing once the clerk of court has received your state and federal background check. Once the court has entered the final judgment of name change, attorney Sandy T. Fox will provide you with certified copies so that you can change your bank accounts, drivers license passport and all other important documents.
